Physical Card - Must be completed by your doctor before beginning practice with your team. |

Emergency Information - Must be completed by your parent or guardian before beginning practice with your team. |

Transfer Approval - Must be completed by the student's former school principal, and approved by the IHSA, before student may participate in athletics at NCCHS. |
